1. Scan
Use a smartphone camera or QR scanner app over this QR code to open the SafeScan demonstration.
2. Submit
Read through the site induction questions and enter information in the fields where required. Submit the form to your nominated email address.
3. You're done!
Check your nominated email address to view the submitted site induction form. This is an example of what you could receive live from your workers on site too!

Tailor a digital safety solution for your business
You can choose one QR code for everything you need or tailor your QR codes to match your business preferences. Each QR code comes with a set group of forms (4 or 6).
Depending on your business need, you can:
- Buy one QR code for each site supervisor (so they can handle their worker site forms)
- Get a different QR code for each site
- Use one QR code across multiple sites.

SafeScan can be used to access one or more of the following site activities:
Contractors, Visitors and Suppliers use this form to complete and record on-site safety inductions. The questions on this form are designed for trade contractors that will be working on your site, and also for Visitors and Suppliers.
This form is to be completed on their first day working/visiting each site, before they come onto the site.
This form facilitates hazard identification.
To be completed as required to advise the business of any site hazards identified on site that require rectifying.
This form facilitates incident notification and the completion of an incident report.
To be completed when an incident or "near miss" occurs or a worker is injured.
This form facilitates SWMS & Risk Acknowledgement
To be completed as required prior to starting a High Risk Work, after reviewing the Safe Work Method Statements Template provided by the Builder.
Contractors will be able to review the SWMS, sign them off, make any comments, make further requests for the job and also assess for any other possible risks.
This form facilitates pre-start meetings and toolbox talks.
To be completed where pre-start meetings or toolbox talks are held by either the business or contractors.
Your SafeScan forms can be linked to your own cloud-based safety management system or safety documents (managed by you and your team). You will be able to provide more information to workers on-site by connecting your SafeScan forms with these cloud-based systems:

- Dropbox
- Google Drive.
